Service Agreement Summary

Scope of Services

Peña Family Care Services LLC provides private-pay, non-medical home care services only. Services focus on personal care and daily living assistance, including hygiene support, mobility assistance, meal preparation, light housekeeping related to the client, companionship, and general safety monitoring.

Medical care, skilled nursing services, diagnosis, treatment, and emergency response are not provided.

Private-Pay Requirement

All services are provided on a private-pay basis. Peña Family Care Services LLC does not bill Medicare, Medicaid, or any third-party payer. Payment responsibility rests with the client or the designated responsible party.

Scheduling and Minimum Hours

Services are scheduled in advance based on caregiver availability. A minimum number of hours per visit may apply. Requests for changes or additional services are subject to availability and reasonable notice.

Cancellations

Advance notice is required for cancellations. Late cancellations may be billed in accordance with agency policy as outlined in the full Service Agreement.

Payment Terms

Invoices are issued according to the agreed billing cycle. Payment is due as specified in the Service Agreement. Failure to maintain timely payment may result in suspension or termination of services.

Client Responsibilities

Clients and authorized representatives are responsible for providing accurate information, maintaining a safe home environment, and coordinating all medical care and emergency planning with appropriate healthcare providers.

Termination of Services

Either party may terminate services in accordance with the terms outlined in the full Service Agreement. Immediate termination may occur for non-payment, unsafe conditions, or violation of agreement terms.

No Emergency Services

Peña Family Care Services LLC does not provide emergency services. In the event of a medical emergency, call 911 or seek immediate medical assistance.

Governing Agreement

This summary does not replace the full Service Agreement. Clients and authorized representatives are required to review and sign the full Service Agreement prior to the start of services.
